The inquiry form is not enabled. Please use direct email for general nonclinical inquiries. You can contact info@cloudbasedmedicine.com using the email link on the Contact page.

Inquiry details

When sending is activated, the form sends your name, email, selected audience and service, optional organization and subject, contact preference, conditional phone number, and nonclinical acknowledgment through a Cloudflare Worker to info@cloudbasedmedicine.com. The visitor email is used as Reply-To. There is no inquiry database, records upload, or attachment intake.

Spam protection

The active form uses Cloudflare Turnstile and server-side verification. Rate limiting uses secret-key hashes of the request IP and email address. The application does not log inquiry contents, visitor email addresses, IP addresses, or verification tokens. Cloudflare and the mailbox provider may process operational and email metadata.

Your device and email

Entered details remain in the form if sending fails. The form does not save them in browser storage. Direct email uses your email application and provider. An accepted email request does not confirm inbox arrival or book an appointment.

Nonclinical inquiries only

Do not send medical records, identifiable health information, or other confidential patient information through the form or the general inbox. Records-based reviews remain unavailable until a suitable secure intake process is ready.
